Luthercare is happy to announce that we have partnered with Phoebe Pharmacy to offer a COVID-19 vaccination clinic to our Independent Living residents at St. John’s Herr Estate and Luther Acres! The clinics are expected to be held by the end of the month, pending the reception of the vaccine as scheduled through the federal government. Resident consent forms must be completed and returned.
For Luther Acres residents
Luther Acres residents who wish to receive the vaccine must complete the following steps by Monday, February 15. Download and complete the resident consent form here. Consent forms are also available at our Muhlenberg and Towne Center Reception areas.
Once the forms are completed, please return them to our Support Service Staff, and they will assist you with making copies of your insurance cards. You will then be added to our waiting list, and we will notify you once the clinic date and time is confirmed. Included with the consent forms is an instruction page, example of how to complete the consent form and a list of frequently asked questions. We hope that this information helps to answer your questions and to ensure a successful vaccination clinic.
For St. John’s Herr Estate residents
St. John’s Herr Estate residents who wish to receive the vaccine must complete the following steps by Friday, February 12. All residents have received a copy of the resident consent form. To download and complete the resident consent form, click here.
